Not actually scuba, but freediving: Shallow water blackout. The lesson I learned was not so obvious to me, but obvious in hindsight. I was taught that the primary cause of shallow water blackout is hyperventilating and that as long as you take slow, deep breaths and calmly descend/swim (low oxygen use, allows CO2 buildup to be an accurate "need-to-breathe" indicator) you'll be fine.
